Established in 2010, our facility has over a decade and a half of experience serving seniors and their families with integrity and heart. We offer both private and semi-private rooms, designed for comfort and dignity. We keep our community intentionally small — with no more than 8 residents at a time — so that every individual receives the attention and quality care they deserve. With two caregivers always on duty, we proudly maintain a high staff-to-resident ratio, ensuring round-the-clock support and peace of mind for families.

Daily life at Green Meadows includes engaging activities, home-cooked meals, and the flexibility to prepare individualized meals for residents with specific dietary needs. Our sunny activity room, also known as the sunroom, is a favorite gathering place for games, crafts, and social time.

For residents with mobility needs, we offer safe two-person transfers using a Hoyer lift, handled by trained staff. We prioritize safety, respect, and independence — working closely with families and healthcare providers to meet each resident’s unique needs.

What truly sets us apart is that we are family-owned and operated — and the same family who provides care also lives on-site in the lower level of the home. This unique arrangement allows us to be fully present, responsive, and deeply connected to our residents’ well-being every single day.

Our Services

At Green Meadows Assisted Living, we offer comprehensive care tailored to the needs of each resident. Our services are designed to create comfort, support independence, and ensure the highest quality of life in a warm, home-like setting.

🛏️ Comfortable, Fully Furnished Living

  • Every room is move-in ready — complete with furniture, bedding, nightstand, television, and storage

  • Hospital beds with air mattresses are available for residents who spend extended time in bed, helping prevent pressure sores and improving comfort

  • Private and semi-private room options available

  • The entire home is designed for accessibility — including ramp access throughout, a stair lift chair for multi-level mobility, and roll-in showers that accommodate shower chairs with ease

👩‍⚕️Personal Care & Daily Support

  • Two-person transfers, including Hoyer lift assistance

  • Help with feeding and meal preparation

  • Custom meals for specialized diets (low-sodium, diabetic, pureed, etc.)

  • Daily incontinence care, including emergency showers when needed

  • Full grooming services: nail care, shaving, brushing teeth, and more

  • Home haircut services available

💡 Enriching Daily Activities

  • Structured daily activities to promote mental and physical engagement

  • Celebration of all major holidays and residents’ birthdays with decorations, food, and family involvement

  • Gardening opportunities during the warmer months

  • On-site sunroom serves as a dedicated activity space

  • All activity supplies provided by the home

🧻 Supplies & Wellness Support

  • $150 for monthly incontinence supply

  • We can help purchase specific personal care items for residents as needed (ASAP), to ensure they never go without essentials

  • Medication reminders and wellness check-ins

  • 24/7 caregiver availability with a low staff-to-resident ratio (2 caregivers for a maximum of 8 residents)

  • We pick up medications directly from the pharmacy at no extra charge

  • Coordination with external healthcare providers, as needed

  • Level 0: Companion Living

    Rate: $4,000/month ($133/day)
    Ideal for: Residents who are mostly independent & simply want meals, companionship, & a safe environment.
    Includes:

    - Private or semi-private room

    - All meals and housekeeping

    - Activities and community access

    - Light supervision

  • Level 1: Light Support

    Rate:$4,800/month ($160/day)
    Ideal for: Residents who require minimal help with daily routines.
    Includes:

    - Medication reminders

    - Occasional grooming assistance

    - Weekly laundry

    - Light assistance with dressing

    - 24/7 staff presence

  • Level 2: Moderate Care

    Rate:$5,500/month ($183/day)
    Ideal for: Residents needing daily assistance and personal care.
    Includes all Level 1 services, plus:

    - Help with bathing and grooming

    - Light incontinence care (scheduled)

    - Dietary-specific meal prep

    - Assistance with feeding

    - +$150/month incontinence supply credit

  • Level 3: Advanced Assistance

    Rate:$6,300/month ($210/day)
    Ideal for: Residents requiring multiple daily interventions and mobility help.
    Includes all Level 2 services, plus:

    - Two-person transfers / Hoyer lift assistance

    - Full incontinence care (scheduled and as-needed)

    - Emergency shower response

    - Full grooming (nails, shaving, teeth)

    - Frequent staff check-ins

  • Level 4: Full-Support Care

    Rate:$7,500/month ($250/day)
    Ideal for: Residents with complex care needs and very limited mobility.
    Includes all Level 3 services, plus:

    - Detailed, individual care planning

    - Coordination with hospice or outside providers

    - High attention from caregivers

    - Custom health & mobility care routines

  • Extra Services
